As a Supportive Care Navigator, you'll be part of the Leukaemia Foundation's Triage & Navigation team, playing a vital role as the first point of contact for people affected by blood cancer. Handling inbound referrals and phone calls, you'll conduct comprehensive intake assessments to offer initial support and connect clients to relevant information and ongoing supportive care.

This is a fantastic opportunity for someone passionate about client care who wants to make a genuine difference for people navigating a blood cancer diagnosis, working collaboratively with healthcare professionals to triage, coordinate and facilitate access to the Leukaemia Foundation's full suite of supportive care services.

What you'll be doing

• Receive and manage all inbound referrals and phone calls from individuals and healthcare professionals relating to supportive care services
• Conduct thorough risk assessments and supportive care needs assessments, identifying and prioritising the needs of clients to determine the most relevant services
• Provide clients with accurate information regarding blood cancer and supportive care services, including relevant external supports
• Triage, coordinate and facilitate access to supportive care services across the Leukaemia Foundation's service offerings, including digital, 1:1 and practical supports such as financial assistance
• Initiate internal and external referrals where required, working collaboratively with healthcare professionals
• Where relevant to the service location, provide responsive support to accommodation residents during periods of reduced onsite Supportive Care Case Manager availability
• Maintain accurate and up-to-date healthcare records, ensuring compliance with organisational policies, regulatory requirements and health service standards

What we are looking for:

• Tertiary qualification in social work, nursing, or an allied health profession
• AHPRA registration or membership of the respective professional body
• Comprehensive understanding of blood cancer and its impact on clients
• Strong interpersonal and communication skills, with the ability to provide empathetic and compassionate support over telephone and digital modalities
• Excellent organisational and time management skills, with the ability to prioritise and manage multiple tasks
• Demonstrated ability to work effectively both independently and as part of a multidisciplinary team
• High level of emotional resilience and ability to handle sensitive and distressing situations

It's a bonus if you have:

• Experience in haematology/oncology settings and/or supportive care in cancer services
• Training in counselling or psychosocial support
